CommunityCARE connects Louisiana Medicaid enrollees to a doctor, clinic or medical practice that serves as their Primary Care Provider (PCP). That PCP manages the care of the CommunityCARE member by:
- providing preventive and acute care;
- teaching members about healthy choices;
- referring to specialists when needed;
- admitting to the hospital when needed;
- sharing medical information necessary to a member’s care with authorized health care providers, such as the emergency room or specialists; and
- ensuring members have medical coverage 24-hours-a-day, seven-days-a-week.
Health care coordination through CommunityCARE is only available to Louisiana Medicaid enrollees who meet certain qualifications. There is no application for CommunityCARE. Medicaid enrollees who qualify are automatically enrolled, and the majority of Medicaid enrollees qualify.

CommunityCARE PCPs are reimbursed for the services they perform for members on a fee-for-service basis. In addition, PCPs are also paid a monthly fee for managing the care of each CommunityCARE member linked to their practice.
